Patient Centered Medical Homes in Colorado:
"Past, Present and Hopes for the Future"
The federal government, many state governments, insurance carriers, employers, patient advocacy groups and various provider groups are looking to Patient Centered Medical Homes as part of the solution to care for the hoards of aging Baby Boomers in a safer, more efficient and cost effective manner. Groups such as CCGC, here in Colorado, are managing pilot projects to demonstrate the feasibility and impact of the PCMH. However some physicians see them as too complex and challenging to implement in the world of understaffed and under compensated primary care. Is there any resolution? How are they faring in Colorado? Where is the money coming from? Please join CMCC and our expert panel at Children's Hospital on May 11, 2010 for some answers.
Julie Schilz, B.S.N. M.B.A. of Colorado Clinical Guidelines Collaborative (CCGC)
The Manager of the Pilot for Colorado Multi-Stakeholder PCMHs, and a frequent guest lecturer on the national circuit, Julie will give us an overview of PCMH from the Past into the Present. She'll review the definition and the origins of the PCMH concept as well as its development in Colorado and across the country. She'll update us on the CCGC pilot and she'll describe how "PCMH" fits in with both Accountable Care Organizations (ACOs) and the Patient Protection and Affordability Act.
David Ehrenberger, M.D.
Medical Director Avista Adventist Hospital and integrated Physicians Network
A noted leader in the Denver-Boulder physician community as well as a practicing family physician, Dr Ehrenberger will share a physician's perspective of PCMH in the Present day. He'll talk about the process a physician group goes through to get qualified. And he'll provide examples of how a physicians practice changes with the change from "business as usual" to one with a Medical Home model. Is care actually delivered differently? How so? Is this just a designation and it's care as usual? What are the pros and cons from a physician's perspective?
